Caching controls and JSON stats output

Tasks:

• Add cache_mode (off|deterministic|always).
• Add cache_key_salt override.
• Add cache --stats --json output for CI and scripts.

Acceptance:

• Cache behavior is configurable and stats can be consumed programmatically.
