Sorry for the late reply.

Which SAML provider are you using? this seems like an issue from that side spitting out bad SAML responses

I must admit I do need to find time to redo this module from scratch as it is not my best work by far and has not been updated in 2 years but have not had the time to sink into this as its a very complex (or was at the time if I recall) module
