AI workflow files are local-only and skip PR review

The repo still ignores .codex/ and .cursor/, so skill and hook changes stay local instead of being reviewable like normal project files. The current make-closed-issue flow also closes work on commit instead of through a reviewed PR, which no longer matches the branch-per-change workflow.
