fix: Resolve licence ambiguity: add a proper LICENSE file

The LifeLine-ICT README states 'MIT, Apache' as the licence but no LICENSE file exists in the repository root. A GitHub repository without a LICENSE file is legally 'All Rights Reserved' by default, directly contradicting the project's open source intent.
