Wrong index files in resulting CASC storage #5

Currently the code generates index files using the old format (one index per data file) instead of the new format that always has 16 index files. This causes games to reorder the entire storage on first launch, which can take a while.

(Posting this as a heads up, not sure when I'll get around to fixing it, as this tool was intended for educational use anyway)
