Improve Documentation

I'm sure nobody is going to enjoy this :P But we do need better documentation.

I suggest using Github's Wiki.

We'll also want to figure out what goes in the README.md and what goes in the Wiki. Ideally I'd like the README to be short and sweet with all the gory details in the Wiki, including things like troubleshooting guides, FAQs etc.
