Add other shortcodes to TinyMCE button

This issue is currently blocked by #22

Currently, the button on TinyMCE only has actions to add [show_as_form] and [as what= ... ] shortcodes.

Discussion points

• What is an intuitive UI for creating these shortcodes?
• Should the screen for [block] include an HTML editor? Or should you have to apply formatting with the main editor after you get the basics generated?

I was thinking, if nothing else just a sample of the code. But it would be cool if it first asked them which field to base this block code on for example.
