I guess the terminal was crashing a few times, idk what other logs to look for. Combined with a run of false positives on input detection and "focus terminal" not doing anything

Logs show that Process had no disconnect runners but was an orphan so the previous VS Code session didn't cleany detach from the terminal. So the previous window crashed or was force-killed rather than gracefully shutdown.

Hm, so it tried to reuse a terminal, but it was in a bad state due to a window crash? I haven't seen it again today but windows will crash, so we need to be resilient to that.
