Using national identifiers

I don't think that the principle employed here https://nhsconnect.github.io/fhir-policy/national-services.html#FHIR-NAT-01 is being followed for (e.g. NHS Numbers).

I can see this was incorrectly applied in the Appointment example, if there are any other instances please let me know. If not I'll close this ticket.

Need to check if all the profiles should use demographics url or fhir.nhs.uk? I am not sure.
I think as per latest pull request it should be changed everywhere to - patient identifier system url to fhir.nhs.uk/Id/nhs-number.
There is inconsistency on the site still. Keep this open so we can discuss please. I am adding in a number of other new issues to be addressed in a new NWR.

Appointment resource references demographics.spineservices.nhs.uk. And also in patient profile and appointment registry.
