feat: add PDS status line integration

Problem

Users can't see PDS state at a glance without explicitly running a command.

Solution

Configure Claude Code's statusLine setting to run a PDS status script that shows:

• PDS v5.0.0 | phase: dispatch | tier: med | agents: 3 (during swarm)
• PDS v5.0.0 (during non-swarm)
• Telemetry indicator if enabled

• install.sh — configure statusLine in settings.json

Partially implemented:

What exists:

• hooks/scripts/status-line.sh: shows 'PDS v{VERSION} | {PHASE} | {TIER}' during swarm, 'PDS v{VERSION}' otherwise, plus telemetry indicator
• install.sh: 'statusLine' is in the PDS-managed keys list (install.sh:293) so it will be propagated to user settings IF it's in the source settings.json

What's missing:

• .claude/settings.json does NOT have a 'statusLine' key — so the install script will never actually write it to ~/.claude/settings.json
• Fix: add '"statusLine": "${CLAUDE_PLUGIN_ROOT}/hooks/scripts/status-line.sh"' to .claude/settings.json
