Context
The recovery-fallback PR (#3) set the MCP serverInfo string to 0.4.0, but v0.4.0 is already the #273 reset release. Correct the cosmetic version string to 0.4.1 so it matches the tag this lands under. composer.json has no version field — packagist derives from the git tag, so this string is informational only.
