Add discard method to explicitly remove it on entry deletion. This puts everything in place for a file storage implementation of the local state (we currently only cover PDO / InMemory).

Use fractional seconds for password failure time and grace use. We need this to properly reconcile between multiple servers (primary + replica). The draft behera doc explicitly notes this.

Add a helper to configure a replica's bind identity ACL rules. Update the docs to note the way password policy state is forwarded and handled locally.
