Document MCPJam MCP server tools

Added documentation for the hosted MCPJam MCP server and its available tools. This update documents the new getWorkspaces and getOrg discovery tools that enable MCP clients to find organization and workspace IDs without using the web app.

Changes

• Updated docs/hosted/overview.mdx to add a new "MCPJam MCP server" section documenting:
  • Server URLs (production and staging)
  • OAuth 2.0 authentication flow
  • All four available tools: whoami, getWorkspaces, getOrg, and doctor
  • Example usage patterns
