This PR introduces a new CHANGELOG.md file in the docs/ directory to track and document all notable changes to the CircleCare project. The changelog follows the Keep a Changelog format and adheres to Semantic Versioning. It includes an "Unreleased" section for upcoming changes and serves as a centralized place for users and contributors to understand the project's evolution. This improves transparency and helps with release management.
