This upload is only to update the logo on these forms. I will address other issues with the forms in separate pull requests.

re: membership application and withdrawal request the fonts are different for "Membership Application" and "Member withdrawal request". I don't care much for either font but I think we should choose a font the works for both the "source" odt and the final pdf?
And the emeritus request is still not exported to pdf.

If all of the .odt and .pdf forms are updated and pushed to the cottage14-patch2 I can review them.
Can you take a look at the fonts and make sure that the fonts are the same for both odt and pdf? Maybe I chose the wrong font originally (Comic Sans?).
