Add hidden instant operations list

Summary

• Add hidden surf list-instant-operations command for instant-mode operation discovery.
• Use blacklist filtering for instant-inappropriate operations, currently onchain-sql and onchain-structured-query.
• Include parameter/request/response schema sections in --detail, and mark required params with * in the summary.
