docs: credit amq-squad as a project built on AMQ#135

omriariav/amq-squad is a role-aware agent team launcher built on top of AMQ. It already appeared in our docs only as an anonymous reverse-DNS example string (io.github.omriariav.amq-squad); this names and links it reciprocally: - README "Extension Metadata": turn the example string into a named, linked amq-squad reference. - README: new "Built on AMQ" section crediting amq-squad / @omriariav, framed as the layer above, with an open invitation for other projects. - docs/adr-layer-extensions.md: name amq-squad as a live implementation of the extension contract. Reviewed by Codex (no blocking findings); verified attribution wording against upstream amq-squad source. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>
