Refresh patch 0.1.1 execution status

Summary

• Refresh docs/execution-status.md current status and PM rule to the published patch 0.1.1 evaluation surfaces.
• Qualify older Milestone E/source-only blocker sections as historical traceability instead of current posture.
• Add a validation record/index entry and update the execution-status guard.

Boundaries

• No hosted surface, production, Windows packaged artifact, bundled PDFium, benchmark, ethos-doc, or ethos-rag boundary opens.
• PDFium remains caller-provided.
• Historical validation records keep their at-the-time wording.
