feat: add --watch and rendered preview mode#45

Add an event-driven --watch flag (via notify-debouncer-mini, using native OS file system notifications instead of polling) that reloads open files when they change on disk, and a new Preview mode (`p`) that renders Markdown source close to its final look in the terminal. Update README and the vhs demo to cover multi-file tabs, watch mode, and the preview feature.
