[codex] add record and replay baseline

Summary

• Add the macOS Record & Replay-compatible event-stream MCP baseline and OCU extension commands.
• Add session validation, summarization, skill scaffolding, standalone repo scaffolding, official fixture import/readiness/compare tooling, and audit summaries.
• Add sanitized official/non-recording fixtures plus the required official simple-action-stop successful recording fixture.
• Document the replication plan, official golden capture workflow, handoff, quality status, and history.
