More support for materialize-css v1

Hi @jamesfwz. As promised, a few more changes.

• Updated generator file to include v0 and v1 html structure. I default to v1, but have the v0 available as comments. I know this probably isn't the best approach, so if you have a different strategy, let me know and I can update.
• Updated readme with instructions for how to do the above
