Add --url-param-file for reusable query parameters

Closes #311

Adds --url-param-file for key=value query params with blank line and comment support. File params are loaded before inline params so inline --url-param wins on conflicts.

Validation:

• go test -short ./src/internal/service ./src/internal/cmd
• mage test build
• golangci-lint run --timeout=5m --new-from-rev=origin/main

Note: mage preflight currently fails on unchanged baseline lint issues in cli/src/internal/service/format_table.go, cli/src/internal/service/repeat.go, and cli/src/internal/cmd/mcp.go.
