Explicit feedback forms have near-zero adoption in practice. Removes request_feedback() and submit_feedback() from the public SDK surface. Action observation (report_action) is the right high-quality signal channel. Terminal attribution and turn capture handle the passive cases.

Removes request_feedback() and submit_feedback() from the SDK.
Explicit feedback forms don't get adopted. Action observation (report_action) is the correct high-quality signal channel — it captures what users actually did, not what they said about it when prompted.
Version bump to 1.12.1.
