chore: drop duplicate uppercase PR template

What & why

Terraform already manages .github/pull_request_template.md. An older uppercase .github/PULL_REQUEST_TEMPLATE.md remained in the tree, so macOS (case-insensitive) showed a phantom dirty file whenever the two blobs disagreed.

Remove the uppercase path; keep the Terraform-managed lowercase template.
