feat: harden changelog-synth prompts

Summary

• make the built-in changelog-synth task definition deterministic and repo-aware
• add changelog-specific planning and implementation guidance alongside the existing release-notes specialization
• lock the behavior with prompt/task regression tests and update the published task reference
