fix: normalize commit task prompts

• add commit-normalize task guidance as a standalone prompt section for planning and implementation
• keep the guidance scoped to the active branch or PR and preserve Nightshift trailers
• add regression tests that verify the guidance content and guard against duplicate numbered instructions
