fix: vocabulary count 58 -> 61, add missing fit pack word#15

Conversation
Phase H of the Liminate Phase 1 UX coherence fix pass. SKILL.md said "58-word prose-as-syntax language" in its own intro while its Vocabulary constraint section (correctly) said 61 -- a self-contradiction within one file. Fixed both real-prose claims and the two worked examples (the format-illustration example and the cite/verify usage example) to 61, so no stale number survives even in illustrative code blocks. README.md's session-pack table was missing `fit` entirely (session_pack v0.4.0 added it after this table was last touched) -- stale at "6 domain words" against a "58-word base." Added the fit row and corrected the counts to "7 domain words" / "61-word base," matching what SKILL.md's own Session pack section already documented correctly.
