Merge upstream PRs streaak#149, streaak#165, streaak#168. OpenAI section combines responses, chat/completions, and files endpoints plus a financial-cost note. Fix TOC anchors to match generated header slugs. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>
