Based on turnermm#29, reworked with semantic HTML (fieldset, labels, CSS classes) and a translate_card() helper for suit symbol display. Raw card codes are kept as checkbox values to preserve captcha matching. Add screen.less for plugin styling.

Added some styling to the build-in CAPTCHA like increased readability.
Fixed submit button content to get it from DokuWiki language.
Refactored the functions I've worked on for better readability/usability - no changes in logic outside
format_cards();
