fix: finalize delivery phase candidate provenance and validation

Summary

• restore delivery-phase commits for issue Implement first-class delivery phases for candidate promotion, acceptance, and demotion semantics #218 on top of devclaw-local-dev
• record promoted candidate provenance on successful reviewer promotion into acceptance
• enforce reviewer/tester role validation for delivery queue/active states
• add focused tests for promotion provenance and delivery schema validation
